MoreWhat Do I Need to Open an Account?
For prompt account setup, please provide either an electronic copy (PDF) of your ordering healthcare practitioner’s license that includes the expiration date or a copy of a voided prescription pad. You will be prompted to upload one of these required documents during your account setup process.
Once we have received your completed form and current license, your account will be opened. A confirmation email will be sent to you along with an electronic copy of our Directory of Laboratory Testing Services and a Fee Schedule. Expect to receive a Welcome Packet, with useful ordering information, shortly thereafter!
